Employers may report their new hires by any of the following methods.
- W4 Form Date of hire, contact name and contact phone number must be listed separate, signature date is not a valid date of hire. Please make sure you complete all employee and employer information including lines 10 & 12.
- New Hire Form
- The Internet through a Web page
- Diskette, Cartridges, or Magnetic Tape
- Email or FTP
