      <title>How Long After An Injury Do I Have To Report It To My Employer?</title>

      <description>Note:  In Wyoming a report of injury is not a claim for benefits.  All benefits must be applied for, using the appropriate Division form.&lt;p&gt;You must report your injury to your employer within 72 hours of the accident causing your injury.  &lt;p&gt;You must also report your injury to the Division within 10 days of the accident causing your injury.  This report must be in writing, on an injury report form provided by the Division.  You may obtain the Wyoming Employee Report of Injury form from your employer or any Division office.  This report must be signed, then returned to the Division.</description>

      <title>If I Am Injured On The Job Can I Choose The Doctor Who Treats Me?</title>

      <description>You may choose a primary physician or chiropractor to treat your work&amp;shy;related injury, but you may not change doctors without prior written approval from the Division or a referral from your primary physician/chiropractor.  To obtain a Request For Change Of Health Care Provider forms, contact your claims analyst or any Division office.  When completing the form, you must explain why you want to change doctors.  When approved by the Division, both the prior and new physician/chiropractor will be notified, in writing, of the change.  You may not continue to treat with a prior physician/chiropractor after the Division has approved your request to change providers.  Also, Failure to attend any medical examination scheduled by the Division could result in a suspension of your benefits.</description>

If you do return to work and wages are less than what you earned before the injury, you may be entitled to partial disability. Wage benefits usually will equal approximately 2/3 of the difference between your current earnings and what you earned before your injury, not exceeding the maximum compensation rate for the year you were injured.</description>

      <title>My Employer Has Denied My Claim, What Do I Do?</title>

      <description>If your employer objects to your reported injury and requests a hearing to determine if benefits should be paid in your case, you may request an Interim Benefit while the case is in litigation.  &lt;p&gt;The Interim Benefit will not exceed 3 months of temporary total disability benefits and only one Interim Benefit may be awarded per reported   injury.&lt;p&gt;The Division will send you a Final Determination letter regarding benefits within 60 days of receiving your signed injury report form.  The letter may be issued sooner, but should not be issued later than 60 days.  The Final Determination letter will tell you what benefits, if any, have been approved.       <description>In Wyoming you must apply for Temporary Total Disability Benefits.  A report of injury is not a claim for Temporary Total Disability Benefits.  Application forms may be obtained from your claims analyst or any Division office.&lt;P&gt;You must complete and submit an Application For Temporary Total Disability benefits to your treating physician or chiropractor. Your doctor must then complete the Health Care Provider`s Certification Of Temporary Total Disability benefits form and submit it to the Division.  &lt;p&gt;When approved, Temporary Total Disability benefits payments are the equivalent of 2/3 of your actual gross monthly wages at the time of injury, not to exceed the statewide average monthly wage.&lt;p&gt;Temporary Total Disability benefits payments are not allowed for the first 3 days of disability, unless you are totally disabled for 8 days or more.&lt;p&gt;When the period of temporary total disability is less than 8 days, you will be paid Temporary Total Disability benefits from the 4th day of disability until the day you returned to work.</description>
