      <description>Under Ohio`s our new managed&amp;shy;care program, BWC and the managed&amp;shy;care organizations (MCOs) have made filing a claim easy. Injured workers, employers and health&amp;shy;care providers should report an injury to their assigned MCO. If they choose, the injured worker may now report the injury directly to BWC through the Internet by submitting the First Report of Injury on&amp;shy;line to BWC.&lt;p&gt;Your doctor will take the necessary information and either submit the injury report electronically or fax a First Report of Injury, Occupational Disease or Death FROI&amp;shy;1 form to the MCO within 24 hours.&lt;p&gt;Once you receive medical attention, if you haven`t already, let your employer know you`ve filed a claim. BWC is here to help you throughout the claims process. If you miss eight or more days of work, a BWC claims service specialist (CSS) will call you within one week of your claim being filed with your MCO.</description>

      <description>If a claim is denied, it is often because of a lack of information. BWC has 28 days to issue a decision. If the information received at that time is not sufficient to allow the claim, then the claim will be disallowed. The specific reason for the disallowance should be documented on the BWC order. If you need assistance understanding the order you may contact BWC at the address or number listed below.&lt;p&gt;Once you receive the BWC order, you have fourteen days to file an appeal if you disagree with the decision. The appeal must be in writing and should include your name, the name of your employer, your claim number, the order date, and the reason you are appealing the decision. Form IC&amp;shy;12, Notice of Appeal, is available from any BWC Service Office.  Once an appeal is received the claim will be referred to the Ohio Industrial Commission for a hearing to be scheduled. Other than these exceptions, most injured workers are usually paid for the entire period of time they are off. If an injured worker is off work for three months (90 days) a medical exam may be scheduled. Continued payment of temporary total disability will be dependent on the outcome of the independent medical exam.&lt;p&gt;Compensation rates are based on your earnings prior to the injury taking into account the minimum and maximum rates applicable to the year you were injured.
