      <description>Applications for Resident Agents and Brokers and Applications for Non&amp;shy;Resident Agents and Brokers are available from the DC Department of Insurance Web site. &lt;p&gt;The Department of Insurance and Securities Regulation also governs the licensing of insurers and ensuring continued compliance with DC laws and regulations. An insurer may obtain a Property and Casualty license or a Life and Health license. The Uniform Certificate of Authority Application (UCAA) process is designed to allow insurers to file copies of the same application for admission in numerous states. Each state that accepts the UCAA is designated as a uniform state. While each uniform state still performs its own independent review of each application, the need to file different applications, in different formats, has been eliminated for all states that accept the uniform application.</description>

      <description>Assessment Systems, Inc. (ASI) administers insurance licensing examinations in the District of Columbia. Candidates may make a reservation by: &lt;ul&gt;&lt;li&gt;calling ASI Customer Care at (800) 274&amp;shy;0610&lt;li&gt;faxing the Fax Reservation Form (found in the Candidate Handbook to ASI at (888) 204&amp;shy;6291 &lt;li&gt;visiting the ASI Web site http://www.asisvcs.com &lt;/ul&gt;&lt;p&gt;Candidates should make a reservation by phone at least three (3) business days before the desired examination date or by fax at least four days prior. The examination fee is $75 per exam for combination major lines examinations (Property and Casualty or Life/Accident and Health).&lt;p&gt;The Candidate Handbook is available to assist and guide applicants through the examination and licensing process. It is suggested, that applicants familiarize themselves with this information before registering for the examination(s). Content Outlines are available on the ASI Web site.</description>

If you are terminating an appointment "For Cause", you must include supporting documentation.       <description>Each applicant who applies for a license as an insurance agent or broker shall furnish at the time of filing his or her application a certificate of evidence of satisfactory completion of a pre&amp;shy;licensing course of instruction consisting of 40 hours. Every applicant for licensure shall take the appropriate license examination within one (1) year of completing a pre&amp;shy;licensing course. Self&amp;shy;study courses will be approved for pre&amp;shy;licensing and do not require a supervised examination.&lt;p&gt;Applicants will submit a certificate of completion of forty hours of pre&amp;shy;licensing to Assessment Systems, Inc. (see information on ASI below) along with the license application and appropriate fee in order to take the District of Columbia Licensing Examination to become licensed in the District of Columbia as an insurance agent or broker.</description>
