      <title>How Can I Inform The Licensing Section Of My Change Of Address?</title>

      <description>Licensees are required by Arizona law to promptly inform the Insurance Department of any change in address [A.R.S.  20&amp;shy;302(A) and 20&amp;shy;313] within 30 days of the change. The Insurance Licensing Section maintains up to three different addresses for each licensee:&lt;ul&gt;&lt;li&gt;The address and telephone number of the place of business, &lt;li&gt;A mailing address (if provided by the licensee), and  &lt;li&gt;The address and telephone number of the residence of individual licensees (not applicable for firms and corporations).&lt;/ul&gt;&lt;p&gt;Keeping the Insurance Department apprised of your current addresses is critical! In addition to being required by law (which means a failure could result in disciplinary action), the Insurance Licensing Section relies on this information when it sends application materials, circular letters, official notices and other information. &lt;p&gt;When notifying the Insurance Licensing Department of an address change, please include the following information:&lt;ul&gt;&lt;li&gt;Your full name (or if for a firm or corporation, the full name of the firm or corporation) as stated on your license certificate. &lt;li&gt;The old address (which has changed) and whether the address is your place of business address, your mailing address or the address of your residence. &lt;li&gt;The new address. &lt;li&gt;The name and phone number of the individual that the Insurance Department should contact with any questions concerning your facsimile.&lt;/ul&gt;&lt;p&gt;To fax the information, please address your facsimile to the attention of the Insurance Licensing Section (as shown in the example below).       <title>What Are The Application Fees And License Expiration Dates?</title>

      <description>It is very important that you send the correct license related fees with your license application.&lt;p&gt;&lt;b&gt;FEES FOR RENEWING EXISTING LICENSE AUTHORITY &lt;/b&gt;&lt;br&gt;The license renewal fee is stated on the renewal form.  Regardless of when you send your application, if your renewal application is not &lt;b&gt;&lt;u&gt;received&lt;/u&gt; by the Department on or before the expiration date of your license,&lt;/b&gt; your renewal application must be accompanied by the renewal license fee plus an additional $100.00 for the late renewal fee required by A.R.S.  20&amp;shy;314. Arizona law provides no exceptions to this requirement. You are solely responsible for ensuring that the Department receives your application prior to the expiration of your license. &lt;p&gt;&lt;b&gt;INFORMATION PERTAINING TO NEW LICENSES AND ADDING ADDITIONAL LINES/KINDS OF AUTHORITY FOR EXISTING LICENSES &lt;/b&gt;&lt;ol&gt;&lt;Li&gt;&lt;b&gt;Expiration Dates.&lt;/b&gt;&lt;ul&gt;&lt;li&gt;&lt;b&gt;If you do not presently hold an Arizona insurance license,&lt;/b&gt; your license will expire two years after the last day of the month in which the license is issued. For example, if you apply for a license in February 1999 and your license is granted in March 1999, you will be issued a license with an expiration of March 31, 2001. &lt;li&gt;&lt;b&gt;If you presently hold an Arizona insurance license and an additional license authority is granted,&lt;/b&gt; your additional license authority will expire on the same date as the license authority you have already held (regardless of when you apply for the additional license authority). &lt;/ul&gt;&lt;p&gt;&lt;li&gt;&lt;b&gt;License Fees. &lt;/b&gt;&lt;ul&gt;&lt;li&gt;The license fee for a surplus lines broker authority or a Mexican insurance surplus lines broker authority is $450.00 through June 30, 2000. The fee effective July 1, 2000, shall be $500.00. &lt;li&gt;The license fee for all other license authority is $45.00 through June 30, 2000. The fee effective July 1, 2000, shall be $50.00 &lt;/ul&gt;&lt;p&gt;&lt;b&gt;WHEN ADDING LICENSE AUTHORITY TO AN EXISTING LICENSE, only half the license fee must be submitted when the term of the license authority will be one year or less.&lt;/b&gt; To determine whether you need to submit a full fee or half fee, look at the expiration date on your license. &lt;ul&gt;&lt;li&gt;If the date the Department will receive your application is one year or greater from the expiration date on your license, the full fee is required.&lt;li&gt;If the date the Department will receive your application is less than one year from the expiration of your license, only half the license fee is required. &lt;/ul&gt;&lt;p&gt;&lt;li&gt;&lt;b&gt;Fingerprint Cards and Fees.&lt;/b&gt; If you are a resident of Arizona, you must submit a blue&amp;shy;outlined, matte&amp;shy;finish fingerprint card (Form FD&amp;shy;258) completed in black ink (*see EXCEPTION below). &lt;p&gt;A $24.00 fingerprint card&amp;shy;processing fee must accompany each fingerprint card. &lt;p&gt;&lt;I&gt;EXCEPTION:&lt;/I&gt; If you are adding additional license authority to an Arizona insurance license that is still active, you do not need to submit another fingerprint card or pay the fingerprint card processing fee if you previously submitted a fingerprint card within one year. &lt;p&gt;&lt;table border=1&gt;&lt;tr&gt;&lt;th&gt;EXAMPLES&lt;/th&gt;&lt;/tr&gt;&lt;tr&gt;&lt;td&gt;&lt;ul&gt;&lt;li&gt;If Pat were not presently licensed in Arizona and was applying for surplus lines broker license authority, property &amp; casualty agent license authority, life agent authority and disability agent authority in May 2000, Pat would submit a surplus lines broker license application and an Arizona insurance license application. Pat would pay the $450.00 fee for the surplus lines broker authority and one $45.00 fee for the other license authority (because the Department would receive the application prior to July 1, 2000). Pat would submit one fingerprint card (not two) and would pay one $24.00 fingerprint card&amp;shy;processing fee. Therefore, the total amount Pat would pay would be $519.00 ($450.00+45.00+24.00). Pat`s license would expire May 31, 2002 .&lt;p&gt;&lt;li&gt;If Pat held a life agent license which expires in September 2000 and in August 2000 applied for Variable Contracts Agent authority, Pat would have to pay $25.00 (half the license fee effective July 1, 2000). If Pat had not submitted a fingerprint card and fingerprint card&amp;shy;processing fee to the Insurance Licensing Section within one year, Pat would need to submit these with the application.&lt;/ul&gt;&lt;/td&gt;&lt;/tr&gt;&lt;/table&gt;&lt;p&gt;If you have any doubt as to what fee you need to pay, call the Insurance Licensing Section at (602) 912&amp;shy;8470 or (877) 660&amp;shy;0964 when calling within Arizona but outside the Phoenix Area.&lt;/ol&gt;</description>

      <description>You may choose to mail your application or deliver your application to the department in person. &lt;p&gt;If you are renewing an existing license:&lt;ul&gt;&lt;li&gt;The Insurance Licensing Section must &lt;b&gt;receive&lt;/b&gt; your complete and correct license application materials and accompanying fees &lt;b&gt;on or before the expiration date of your license&lt;/b&gt; for your license to remain in force.&lt;p&gt;&lt;li&gt;If you fail to timely renew your license, the Insurance Licensing Section is permitted by law to renew your license if your complete and correct application materials are received (1) within six months after the expiration of your license and (2) your application materials are accompanied by the renewal license fee &lt;b&gt;plus&lt;/b&gt; a $100 late renewal fee (A.R.S. &amp;sect;20&amp;shy;314). &lt;/ul&gt;&lt;p&gt;If you deliver the application in person, the Insurance Department can review your application during your visit.       <description>The Insurance Licensing Section offers On&amp;shy;the&amp;shy;spot licenses under certain conditions when applicants deliver their license applications in person to the Insurance Licensing Counter for review. &lt;p&gt;You &lt;I&gt;may&lt;/I&gt; be eligible for an on&amp;shy;the&amp;shy;spot license under the following circumstances:&lt;ul&gt;&lt;Li&gt;You must deliver the application directly to the Phoenix or Tucson Office Insurance Licensing Counter between 8:00 a.m. and 4:00 p.m. Mondays through Fridays (except State&amp;shy;observed holidays) and wait while the licensing technician reviews the application and updates the computerized license file. &lt;p&gt;&lt;li&gt;You must not bring more than two license applications for on&amp;shy;the&amp;shy;spot review during one visit. &lt;p&gt;&lt;li&gt;You must have fully and properly completed all application materials, passed any and all required examinations, and achieved license qualifications.&lt;/ul&gt;&lt;p&gt;Certain types of license applications require too much time to qualify for on the spot review.  For example, applications for firms or corporations cannot be processed on the spot. In addition, other types of licenses may not qualify. License applications that are not reviewed on the spot are generally reviewed within one month of receipt of your application and fee payment.</description>

The Phoenix Insurance Licensing Counter is located at 2910 North 44th Street in Phoenix, Second Floor, near the northwest corner of 44th Street and Thomas. On the northwest corner is a Bank of America branch. The office is directly west of the Bank of America in the four&amp;shy;story brown building.&lt;p&gt;Parking is available directly east of the building (between the bank and our building) and on the upper level of the parking structure located to the west of the building. &lt;p&gt;The Tucson Insurance Licensing Counter is located at 400 West Congress Street in Tucson, Suite 152.
