Generally, a report would include the name and address of the child and either the parents or the person(s) responsible for the child, the age of the child, a description of the child`s injuries, and any other related information. There are also certain steps to take in order for the report to be shared with any relevant organizations such as social service and law enforcement agencies, as well as prosecutors` offices.
