How Can I Obtain The Required Forms For Insurance Licensing?

Some agent licensing forms can be retrieved online: http://www.state.tn.us/commerce/agentlic.htm.

If additional forms are needed, send a self­addressed postage paid envelope with form title and number to:

For company admissions; all Life, Health, Property and Casualty companies' applications for admission (certificates of authority) are to be submitted using the National Association of Insurance Commissioner's Uniform Certificate of Authority Application (UCAA). The details of the application, including instructions, forms, application review processes and time lines are found at the NAIC's website: www.naic.org/ucaa

The Primary Application should be used by newly formed Tennessee insurance companies seeking a Certificate of Authority in Tennessee. Those insurers not currently licensed in Tennessee wishing to redomesticate to Tennessee should refer to T.C.A. Title 56, Chapter 11. The Expansion Application should be submitted by all other insurance companies already licensed elsewhere.

In addition to the above requirements, there are Tennessee Specific Forms that must be submitted with the UACC documents.
