Does My Application Have To Be Notarized?
Yes. Applications must be notarized and a company official on record must complete the Sponsor`s Certification with the Insurance Department as being authorized to appoint/terminate agents for that particular company. The official signing the application must print or type their name under their signature. Applications must be notarized within the last 30 days of turning them in.
