What Do I Do After Completing My Advance Directive For Health Care?

Make photocopies of the document. Provide a copy and discuss your choices for end­of­life care with: your family and loved ones, your health care proxy, your doctor, others who might be involved in your care. Keep a copy of your Advance Directive: in your glove compartment, in your travel case, with other important documents (insurance papers, last will and testament). Make a list of where you have copies in case you desire to revoke or revise your directive. Remember to take a copy with you to the hospital. You can revoke your Advance Directive at any time. Your Advance Directive is only operational when you are unable to make your own decisions and two doctors certify it applies.
