What Is The Proper Procedure For Submitting An Application?

Once you have completed all of the pre­licensing educational requirements and passed the qualifying exams, you may then proceed with the application process by:
  1. Contacting the Nebraska Insurance Department and requesting an insurance agent's licensing application (Form DOI­9001);
  2. completing both sides of the application;
  3. make sure it is signed and notarized;
  4. submit a check for the appropriate fee, and the application to the insurance department.
