My License Has Lapsed, How Do I Get It Reinstated?

A license can be reinstated for up to three years after the May 1 anniversary date on which it expired, provided you continue to meet all requirements. You must file a completed application, pay the appropriate fees and show proof of completing continuing education for your license type. Contact the Department directly to request a reinstatement application.
