You may also hire a lawyer to draft the document or documents, or you may write the entire document yourself. If you write the entire document, type it or print so it is legible. You should also discuss your wishes with your family, friends, and health care team. By law, your family, physicians and employees of your health care facility are not allowed to be a witness for a document.

The information on this page is meant to provide a general overview of the law. The laws in your state and/or city may deviate significantly from those described here. If you have specific questions related to your situation you should speak with a local attorney.
