Last Will and Testament (Simple Will)

This form is for single or married people with two or more children. The form used to generate the Children's Trust is broken down into three parts. The first part gathers general information. The second part gathers information relating to appointing a guardian for your child(ren), and the last part gathers information relating to the disposition of tangible personal property.
