Registration No.:	(U.S. Patent Office Registration No. of the Patent)
Issue Date:	(Date registration number was issued to the patent)
Percent of Royalty:	(Percent of the net selling price licensor expects licensee to pay for royalties)
Minimum Royalty:	(Indicate the minimum annual royalty you will accept for the exclusive license of your patent)
In the spaces below, spell out the minimum royalty dollars and cents. For example, if you indicate that you want a $40,000 minimum annual royalty. You would spell out "forty thousand" in the "Dollars" field and leave the "Cents" field blank (because the program will automatically enter the "00"). If you are indicating anything other than a flat dollar amount. You will need to spell out how many cents in the "Cents" field.
