Children's Trust Form

The Children's Trust form is for married people with two or more children. The form used to generate the Children's Trust is broken down into three parts. The first part gathers general information. The second part gathers information relating to appointing a guardian for your child(ren), and the last part gathers information relating to the disposition of tangible personal property.

Once you complete all three parts, a Children's Trust that you can print. After you print the Children's Trust, you will need to have it signed by two witnesses and notorized before taking it to your attorney. Click here for completed sample. Age Child Can Withdraw Half the Trust Assets:

(How old does the youngest child have to be before any child can withdraw half their share of the Children's Trust assets?)

Guardian:

Alternate Guardian:
