• Other On-The-Job Training and Apprenticeship Training Agreement and Standards
  This form is for educational providers who want to provide education/training to Veteran's Administration patients or dependents. (22-8864)
• Certificate of Affirmation of Enrollment Agreement Correspondence Course
  If you have enrolled in a correspondence course, you must affirm your enrollment to the Veteran's Administration with this form before the VA will pay for the class. The VA will not accept this affirmation of enrollment until at least the 10th day after you initially enrolled. (22-1999c)
• Application for Refund of Educational Contributions
  Some GI Bill programs require contributions from the service member while they are in the service. If the member decides to not use the GI Bill they may receive back their prior contributions. Submit this form to the Veteran's Administration to request payment of your prior GI Bill contributions. (22-5281)
• Application for Educational Assistance Test Program Benefits
  Submit this form to apply for the Veteran's Administration Educational Assistance Test Program. (22-8889)
• Trainee Request For Leave
  This form should be used if you are attending a Veteran's Administration sponsored education/training program and need to leave the program because of illness or other hardship. Submission of this form will allow you to continue receiving your VA benefits while you are on leave from the program. (28-1905h)
• Application for Reimbursement of Licensing or Certification Test Fees
  Use this form to request repayment from the VA for licensing or certification fees in relation to your VA education benefits. (VA FORM MAR 2006 22-0803)
• Application for Reimbursement of National Test Fee
  Use this form to request repayment from the VA for a national test taken in conjunction with your VA education benefits. (VA FORM AUG 2007 22-0810)
• Application for VA Education Benefits
  Use this form to apply for educational assistance under the Montgomery GI Bill, Montgomery GI Bill - Selected Reserve, Reserve Educational Assistance Program or Post-Vietnam Era Veterans' Educational Assistance Program. (VA FORM FEB 2008 22-1990)
• Application for VA Education Benefits Under the National Call to Service (NCS) Program
