What Documents and Supporting Information are Required to be Submitted with a Divorce Petition in Kentucky?

A divorce petition must be verified and contain: (1) the age, occupation, social security number, residence, and length of residence of each party; (2) if domestic violence or abuse is alleged then the petitioner must certify the existence and status of any domestic violence protective orders and the petitioner may substitute the party’s attorney’s address as his or her own; (3) the date of the marriage and the place where it was registered; (4) a statement that the parties are separated and the date on which the parties separated; (5) the names, ages, social security numbers, and addresses of any living infant children of the marriage, and whether the wife is pregnant; (6) any arrangements as to custody, visitation, and support of the children and maintenance of a spouse; and (7) the relief sought. (Kentucky States 403.150)
