A divorce petition must contain the required caption, be verified, and set forth: the residence of each party and the length of residence in the state and county; the date of the marriage; the date of the separation; the name, age and address of any living child under 21 or any living child who is incapacitated; a statement as to whether the wife is pregnant; the grounds for divorce; and the relief sought.
