What Are The License Application Procedures?
The specific requirements and forms needed depend upon the type of license you are attempting to acquire. The following information outlines the requirements for various types of licenses. This information is contained in the Licensing Examination Candidate Handbook
An applicant must submit the following to the Hawaii Insurance Division:
Resident General Agent:
- Application for General Agent License Form 401 (one copy);
- Notice of Appointment of General Agent Form 402/404 (two copies), reflecting appointment by an insurer;
- Original ASI passing score report;
- Affidavit Form 4 (one copy);
- Clearance letter If the applicant has previously held any resident insurance license in any state or states other than Hawaii, an original signed and sealed letter of clearance issued by the state insurance department of each state in which the applicant was previously licensed must accompany the Hawaii license application;
- Remit applicable license fees; and
- Corporate licenses:
- Evidence of approved registration with state Department of Commerce & Consumer Affairs Business Registration Division;
- Copy of filed Articles of Incorporation;
- Name a Qualified Designated Representative;
- Letter signed by an officer of Corporation if Designated Representative is not an officer of Corporation, listing job duties, responsibilities and title (position) empowering Designated Representative to operate this agency as if it was their own; and
- Letter signed by the qualified resident individual(s) acknowledging and accepting the duties and responsibilities of this position.

Resident Subagent:
- Application for Subagent License Form 501 (one copy);
- Notice of Appointment of Subagent Form 502 (two copies), reflecting appointment by a general agent or domestic insurer;
- Original ASI passing score report;
- Affidavit Form 4 (one copy);
- Clearance letter Same as #5 under Resident General Agent procedures;
- Remit applicable license fees;
- Corporate licenses Same as #7 under Resident General Agent procedures.
Solicitor:
- Notice of Appointment of Solicitor Form 602 (three copies);
- Original ASI passing score report;
- Affidavit Form 4 (one copy);
- Clearance letter Same as #5 under Resident General Agent procedures; and
- Remit applicable license fees.
Adjuster:
- Application for Adjuster`s License Form 106 (one copy);
- Affidavit Form 4 (one copy);
- Original ASI Adjuster passing score report; and
- Remit applicable license fees.
Nonresident Agent:
