What Must An Agency Do To Become Licensed?

To become licensed as an agency you must file a complete GID 130 form including the appropriate fee to ASI. Once licensed, you will receive an annual renewal notice that must be completed and returned to ASI by January 1st. ASI will send a renewal license after processing is completed.
