What Is The Procedure For Obtaining An Appointment?
To add a new appointment or to terminate an existing appointment, please obtain a copy of the District of Columbia Notice of New Appointment/Appointment Termination. Forms must be filled out and signed by the appointing insurance company. If you are adding or terminating more than three appointments at one time, please photocopy side two of the form and attach together. If you are terminating an appointment "For Cause", you must include supporting documentation. Return forms with appropriate fees to:

All agents must be appointed to do business in DC. Brokers cannot be appointed. Firms may be appointed if desired. Appointment activity reports are sent to companies on a biweekly basis, confirming appointment transactions and requesting additional information to help complete pending transactions. There is a $25 fee for each new appointment payable to Assessment Systems, Inc. Appointments are renewed annually.
