Connecticut Trusts Forms

• Acceptance of Trust/Ex Parte Decree Qualifying Testamentary Trustee
  This form is used to tell the court that you accept the position of trustee for a deceased's trust, and by the court to accept you as a trustee. (PC-265 REV. 3/03)
• Bond Waiver
  This form is used by the heirs of the trust to waive the bond requirements for the trust administrator. (PC-280 REV. 11/98)
• Application to Sell or Mortgage Real Property
  Use this form to request permission from the court to sell or mortgage a piece of real property belonging to the trust. (PC-400 REV. 10/07)
• Affidavit of Financial Responsibility - Personal Surety
  Use this form to give the court a list of all assets and liabilities you have at the time you are appointed to administer a trust. (PC-410 NEW 10/81)
• Inventory
  Use this form to make a list of all assets held in the trust you are fiduciary of. (PC-440 REV. 10/08)
• Fiduciary's Periodic or Final Account
  Use this form to give either a periodic or final accounting of the trust you are fiduciary of. (PC-441 REV. 6/01)
• Probate Bond
  This form is used to certify to the court that you will be responsible for the amount of the bond listed on the form if the fiduciary breaches his/her duty. (PC-480 REV. 10/04)
• Appointment of Judge of Probate as Agent for Service by Non-Resident Fiduciary
  This form is used by a fiduciary of a trust who has moved from Connecticut to appoint the judge of probate to receive all papers relating to the trust. (PC-482 REV. 1/85)
• Receipt and Release of Guardian of Estate
  Use this form if you are a ward in a trust and have now reached the age of majority, to have the guardian released from your trust. (PC-580 REV. 1/03)
