New Hampshire Juvenile Justice System Forms

• Juvenile Waiver of Counsel
  This form should be used by the juvenile and parent/guardian/custodian when the juvenile waives his/her right to an attorney.
• Delinquency Acknowledgment and Waiver of Rights
  This form should be used by the juvenile and parent / guardian /custodian when the juvenile decides to waive his/her rights when admitting the truth of the acts in the delinquent petition.
• CHINS Acknowledgment and Waiver of Rights
  This form should be used by the juvenile and parent when acknowledging the rights that are being waived when pleading True in a CHINS case.
• Juvenile Acknowledgment and Waiver of Right to Jury Trial
  This form should be used by the juvenile and parent/guardian/custodian when the juvenile chooses to waive the right for a jury trial.
• Acknowledgment of License Penalty
  This form should be used by the juvenile in which the judge will review with the juvenile his/her understanding of the license penalty that is a direct result of the conviction.
• Juvenile Diversion Agreement
  This form should be used by both parties involved in the matter. When both parties agree to the juvenile attending a diversion program. This form will also be used to document the terms of the diversion program.
• Notification of Parental Responsibility in Delinquency Case
  This form should be used by the parents or guardian of the juvenile, documenting their responsibilities in the delinquency case.
