Michigan Juvenile Justice System Forms

  • Judgment of Sentence, Commitment to Corrections Department
    This form should be used by the court to document the judgment that has been made against the defendant to be sent to the department of corrections. This form will also document the length of the sentence that has been imposed.
  • Judgment of Sentence/Commitment of Jail
    This form should be used by the court to document the judgment that has been made against the defendant to be sent to jail. This form will also document the length of the sentence that has been imposed.
  • Motion/Order of Nolle Prosequi
    This form should be used by the defendant to file a motion with the court that the case against the juvenile be Nolle Prosequi or "not pursued" by the court. The attached order will be filed by the court with the findings of the court to be that motion is granted with or without prejudice, the motion is denied, the defendant/juvenile will be discharged from confinement of the case, the bind is cancelled and returned or cancelled.
  • Order Committing Juvenile to Family Independence Agency
    This form should be used by the court to document the order of committing a juvenile to the Department of Human Services while on probation. This order will be used to document where the probation will be served.
  • Order Delaying Sentence
    This form should be used by the defendant to request that the court delay the sentence that has been ordered. In order to complete this form, information regarding the reason for the request is to be documented. The defendant must also sign the form stating the understanding that payment for any court costs must be made by a certain date, the vehicle that was used in the offense is to be forfeited and any concealed weapons the defendant may own will be suspended.
  • Order of Acquittal/Dismissal or Remand
    This form should be used by the court to order that the case be dismissed with or without prejudice, the juvenile's motion for dismissal is granted, or the juvenile's motion for dismissal is partially granted with or without prejudice or the case will be remanded to another court.
  • Petition and Order for Discharge from Probation
    This form should be used by the defendant to request that the court discharge the defendant from probation. The defendant must list the reasons that this request is being made, and the court will document the findings regarding the request.
  • Appearance of Attorney/Guardian Ad Litem / Lawyer-Guardian Litem
    This form should be used by the attorney, Guardian Ad Litem or Lawyer-Guardian Ad Litem. This form will be used to document with the court that the named individual will be appearing for the defendant.
  • Authorization for Return of Bond
    This form should be used by the defendant to prove that the bond that has been deposited is satisfied and that the bond has been cancelled or the county treasurer will withhold 10% of the deposit on a 10% cash bond, the remaining deposit will be returned or the deposit shall be applied to court ordered reimbursement.
  • Motion for Alternate Service
    This form should be used by the defendant to advise the court, that service of the Notice was attempted and unsuccessful. This form should be used to request that a different form of service be made by the court.
