- Plea Form with Advisory
This form should be used be a defendant, to document the defendant's understanding of the rights he/she are giving up by pleading guilty or no contest to the charges brought against he/she. This form will also be used to document the plea of no contest or guilty with the court. - Notice of Appeal
This form should be used by the defendant to notify the court that an Appeal of an Order has been filed. This form will document that the order that has been made by the court should be continued to be enforced with any modifications that may be made by the court. - Order Appointing Attorney for Appeal
This form should be used by the defendant to document with the court the name of the attorney that has been appointed by the court to handle the appeal. - Challenge to Order for Execution of Adult Sentence
This form should be used by the juvenile offender to challenge the court for the reasons for the order to execute the adult sentencing. This form will also document that the juvenile has the right to a hearing on the challenge, the right to be heard and to be represented by an attorney. - Request for Review of Sentence
This form should be used by the juvenile offender to request that the court review the sentence that has been imposed. - Motion to Modify Sentence
This form may be used by a parent, commissioner or the court, to request that the court modify the sentence that has been imposed. This form may be used within 60 days of when the sentence has been imposed unless the motion has been made by the commissioner, then the modification may be made at any time. - Juvenile Information Sheet
This form should be used by the juvenile offender or their attorney, to provide personal information regarding the juvenile and the juvenile's family members. This form is required by the Clerk of Courts to initiate a juvenile docket sheet. - Order Appointing Attorney
This form should be used by the juvenile offender or their attorney to document the name of the attorney that will be representing the juvenile throughout the court proceedings.
