• Complaint
  This form is used to file a complaint against a juvenile offender. (JUV-2)
• Petition (Delinquent/Unruly)
  Use this form to have the court declare a juvenile to be a delinquent. (JUV-3)
• Summons and Process
  This form is used by the court to notify a juvenile and his/her parents that they must appear in court. (JUV-7)
• Subpoena
  This form is issued to notify you that your presence as a witness is required in court on a given day and time. (JUV-8)
• Subpoena to Produce Documents or Other Evidence
  This form is used to notify you that you are required to appear in court and bring certain documents with you. (JUV-9)
• Notice of Transfer to Adult Criminal Court
  The court will use this form if it is considering the transfer of a juvenile to an adult criminal court for an alleged offense. (JUV-10)
• Application for Bond
  Use this form to request that the court release a juvenile to your custody on a bail bond. (JUV-11)
• Affidavit and Warrant
  Use this form to give an affidavit to the court requesting a warrant be issued for a juvenile. (JUV-12)
• Order for Detention
  The court uses this form to issue its ruling on whether a juvenile should be held until proceedings against him/her take place or release the juvenile. (JUV-13)
• Adult Commitment Order
  The court uses this form to issue its ruling on sending a juvenile to adult court and being held for bond as an adult. (JUV-14)
