This Affidavit- Inactivation of Child Support Account should be used by the County Clerk, swearing under oath that a diligent effort has been made to locate the obligor, but the he/ she can not be found. - Affidavit of Arrears
This Affidavit of Arrears should be used to prove to the court the schedule of support payments and balances due to the Obligee by the Obligor as ordered by the court. - Civil Contempt Order (Non-Support)
This Civil Contempt Order will be used by the Court to document that the Defendant has been found/not found in contempt of Civil Court for not paying child support. This form will be used to Order the payment of child support and/or the arrest of the Defendant, if the defendant has been found to be in contempt. - Designation Form
This Designation Form will be signed by the Non-Custodial Parent acknowledging that a portion of child support payments should be used for court fees. - Directions for Payments of Child Support or Alimony
This Directions for Payments of Child Support or Alimony form will be used to list all that is required to make child support and/or alimony payments. This form will list what the Court has ordered including: Frequency of payments, to whom the payments will be made, and whether wage withholding has been ordered. - Financial Declaration
This Financial Declaration will be used to document the finances of both parties. - Information Transmittal Form
This Information Transmittal Form should be used to inform the County Clerk of any information regarding address, employment, or County change that you may have. - Interim Child Support Order
This Interim Child Support Order will be used to order the defendant to pay child support in the amount specified as well as 5% for court fees. - Motion to Dismiss Support Payments
Use this Motion to Dismiss Support Payments to request that the court terminates child support payments. This form will document to the court, why child support payments should be terminated such as, child is emancipated, or couple has reconciled. - Motion to Transfer Support Collection Responsibilities
