New York Child Support Forms
- Petition (For an Order Upon Support Agreement)
Use this form to request that an order of child support be entered by the court and that the respondent be required to exercise the option of additional coverage for health insurance in favor of his/her spouse and child(ren). (Form 4-1). - Summons
The summons requires that the defendant appear in court on the scheduled day and provide the court with proof of his/her income and federal income tax returns. (Form 4-1a). - Summons (Non-Resident)
Use this child support form when the defendant is in another state. The summons requires that the defendant appear in court on the scheduled day and provide the court with proof of his/her income and federal income tax returns. (Form 4-1b). - Order Upon Support Agreement

This form is used by Social Services to request an order of support directing the respondent to pay fair and reasonable child support, and that he/she be required to exercise the option of additional coverage for health insurance in favor of his/her spouse and named child(ren). (Form 4-3a). - Remand Commitment (Undertaking)
This form is an order from the judge directing the sheriff to arrest a person who has failed to pay child support and to appear on the date summoned. (Form 4-4). - Undertaking for Support - Cash Deposit
Use this form to agree to pay child support by cash deposit. (Form 4-5a). - Temporary Order of Support & Referral to Support Magistrate
The form is used by the judge to direct a person to pay child support to the petitioner or to a support collection unit a rendered amount and giving him/her a payment schedule. (Form 4-6). - Referral to Support Magistrate & Temporary Order of Support
This form is used by the judge to order a respondent to pay child support and to order the civil officer to enforce the order of support if needed. (Form 4-6a).
