• Child Support Formula
  This form provides information on ordering a computer program or pamphlets that show how child support orders are calculated in Michigan.
• Motion for Lien
  This form should be used by the Defendant or Plaintiff to request that the court place a lien on the property that is owned by the individual who is overdue in child support. This will ensure that there are no longer overdue support payments.
• Change in Personal Information
  This form should be used by party and minor child, for minor child no longer living with custodial parent or for party only. This form will document any changes that need to made regarding name, address, new employer, social security number or Health Care Insurance provider.
• Employer's Disclosure of Income and Health Insurance Information
  This form should be used by the named individual's Employer. The information provided will be used for the purposes of administering, enforcing and complying with laws regarding child support.
• Employment Status Disclosure
  This form should be used by an individual to update court records with employment information. This form should also be completed by the individual's employer to verify the information given.
