• Order/Notice to Withhold Income for Child Support
  Employers are obligated to garnish the wages of employees who are subject to a wage withholding court order for child support payments. This form is used to notify an employer who must withhold money from an employee's paycheck. (JD-FM-1 Rev. 1/05 (OMB 0970-0154)
• Instructions to complete the Order/Notice to Withhold Income for Child Support
  Employers are obligated to garnish the wages of employees who are subject to a wage withholding court order for child support payments. These instruction have been prepared to assist in completing the Order to Withhold Income for Child Support. (JD-FM-1 (i) Rev. 7/05)
• Application for Contempt Order, Income Withholding, and/or Other Relief
  If you are a recipient of child support payments and the other party is delinquent use this form to apply for an order of contempt of court, income withholding, or order to work, for failure to pay child support or other child care expenses. (JD-FM-15 Rev. 4-05)
• Notice and Claim Form Support Income Withholding
  If you are a recipient of child support payments and the other party is delinquent or refuses to pay use this form to apply for an income withholding order. (JD-FM-68 Rev. 10-99)
• Notice to Nonappearing Obligor of Income Withholding Order
  If an income withholding order is granted by the court to enforce an earlier child support order the defendant must be served with this form. (JD-FM-70 Rev. 9-99)
• Advisement of Rights
  If you are subject to an income withholding order this form advises you of your rights in your child support case. (JD-FM-71 Rev. 6-03)
• Appeal from Family Support Magistrate
  You have a right to appeal an adverse child support or child custody judgment by a Family Support Magistrate. Submit this form to request an appeal be heard by the Superior Court. (JD-FM-111 Rev. 12-05)
• Contempt Proceedings Upon Failure of Payer of Income to Comply with Withholding Order for Support
  If you are a recipient of child support payments and the other party is delinquent or refuses to pay you may apply for a contempt order. Submit this form to apply for a contempt order. (JD-FM-124 Rev. 11-01)
• Order to Maintain Health Insurance for Minor Child(ren)
  A child support order may also require a parent to pay for health insurance. This form is used to inform a non-custodial parent that he/she is ordered to make health care premium payments for the children involved. (JD-FM-125 Rev. 5-06)
• Case Input Record Non IV-D Income Withholding
  Connecticut requires income withholding in all child support cases. You must submit this form to enable the state to notify the other parent's employer of their duty to withhold income. (JD-FM-150 Rev. 12-05)
