California Child Support Forms

• Child Support Information and Order Attachment
  This form is used by the court to show how it arrived at the amount of child support to be levied and to order that specific amount to be paid. (FL-342)
• Non-Guideline Child Support Findings Attachment
  This form is used by the court when the amount of child support determined to be paid does not follow the standard guidelines. (FL-342(A)
• Stipulation to Establish or Modify Child Support and Order
  Use this form if both parties in a divorce action agree on the amount of money to be paid either establish or modify a child support order. (FL-350)
• Statement for Registration of California Support Order
  Use this form to state to the court that you are applying to register a California Support Order. (FL-440)
• Request for Hearing Regarding Earnings Assignment
  Use this form to request a hearing on an earnings assignment and state your reasons why it should either be set aside or modified. (FL-450)
• Stay of Service of Earnings Assignment Order
  Use this form to request that the court stay the service of the earnings assignment order in this case and state your reasons. (FL-455)
• Qualified Domestic Relations Order for Support (Earnings Assignment Order for Support)
  This form is sent by the court to the obligor's employer stating that the employer is to withhold part of the employee's earnings as payment of child support. (FL-460)
• Attachment to Qualified Domestic Relations Order for Support
  This separate statement of confidential information lists the social security number of the Employee for whose benefit spousal or family support is being withheld under the Qualified Domestic Relations Order for Support. (FL-461)
• Application and Order for Health Insurance Coverage
  Use this form to apply to the court to require that health care coverage be provided to your children by the spouse who is paying child support. (FL-470)
• Employer's Health Insurance Return
  This form should be completed by the obligor's employer and returned to the local child support agency listing the health care coverage the employee has with the employer. (FL-475)
