• Application for Child Support Services
  Use this application form to apply for child support services from the Alabama Department of Human Services Child Support Enforcement Division. (DHR CSD 570, DHR CSD 1491)
• Custodial Party (CP) Case Information Worksheet
  Custodial parents seeking child support should submit this form with the application for child support services. Your case worker will use it to develop your file.
• Child Support Obligation Income Statement/Affidavit
  In child support cases both parties must provide the court with information about income. You must submit this income affidavit to the court prior to your child support hearing.
• Employers Child Support Withholding Template
  This form is for employers who are required to withhold money from employees because of a court order. This form is to be used to list each employee subject to child support withholding.
• Non-Custodial Parent Child Support Payment Coupons
  Parents under a child support order should use these payment coupons when submitting child support payments to the state.
