• Affidavit for Citation for Contempt of Court (Visitation- for use by Non-Custodial Parent)
  This Affidavit for Citation for Contempt of Court should be filed by the Non-Custodial Parent. This Affidavit swears under oath that the Custodial Parent is in contempt of the visitation order.
• Order of Contempt (Visitation)
  This Order of Contempt (Visitation) will be used by the Petitioner to inform the Court of his/her belief that the Respondent has failed to comply with the terms of the Order of Protection.
