• Interstate Compact on the Placement of Children Request
  This child custody form is used by the issuing state to request the placement of a child in another state either for foster care or adoption. (DPHHS-CFS-019C ICPC 100A REV. 8/2001)
• Interstate Compact on the Placement of Children Report on Child's Placement Status
  This child custody form is used to inform all interested agencies what the status of the child's placement is, whether it is permanent or not. (DPHHS-CFS-019D ICPC 100B REV 09/2001)
