Michigan Child Custody and Visitation Forms

• Motion and Order to Show Cause for Contempt (Custody/Parenting Time)
  This form should be used by a friend of the court, to request that the court to issue an order to show cause why the named individual should not be held in contempt of court regarding the parenting or custody time.
• Motion Regarding Custody
  This form should be used by either the Plaintiff, Defendant, or Third Party requesting that the court order a change in the existing custody order. This form will document the reasons why the change needs to be made and that there is an agreement between the individuals regarding the changes to custody.
• Motion Regarding Parenting Time
  This form should be used by either the Plaintiff, Defendant, or Third Party documenting that the Parenting Time agreement has been violated. This form will be used to describe the violation and document the change that needs to be made whether it be to establish parenting time or change parenting time.
• Response to Motion Regarding Custody
  This form should be used by the Plaintiff, Defendant, or Third Party documenting whether or not he/she agrees that circumstances have changed regarding custody and whether or not he/she agrees with the changes that are being requested.
• Response to Motion Regarding Parenting Time
  This form should be used by the Plaintiff, Defendant, or Third Party documenting whether or not he/she agrees that circumstances have changed regarding parenting time and whether or not he/she agrees with the changes that are being requested.
• Motion to Permit Foreign Travel
  This form should be used by the Plaintiff or Defendant to request that the court grant permission for the individual to take the named children out of the state of Michigan. This form will require the reason for the trip out of state.
