Hawaii Child Custody and Visitation Forms

• Proposed Parenting Plan
  Use this form to outline for the court your plans for custody and visitation of your child(ren).
• Custody/Visitation Statement
  Use this form to give the court relevant information about the current custody and visitation of your child(ren).
• Custody Evaluation Appointment Order and Restraining Order
  This form is used by the court to order that a Custody Evaluator investigate and report his/her findings to the court on child custody.
• Custody Evaluator's Acknowledgment of Appointment and Certification of Minimum Requirements
  This form is completed by the person the court designates as a Custody Evaluator and gives his/her qualifications.
• Notice that Custody Evaluator has Declined Appointment
  This form is completed by the person the court asks to be a Custody Evaluator and states that he/she declines the appointment.
• Notice of Completion of Custody Investigation
  This form is submitted to the court by the Custody Evaluator and states that he/she has concluded the investigation for child custody.

  This form is used by the Custody Evaluator to certify their annual training to the court and their willingness to serve again.
