- Notice of Application
This form should be used by an inmate to notify the Office of Victims Services that an application to the Board of Pardons, Board of Parole, Department of Corrections, Sentencing Court, Sentencing Review Court, Court for the exemption of from sex offender requirements, or Court for an Order restricting the dissemination of sex offender information. - Confidential Request for Notification of Status Inmate
This form should be used to request that the court notifies the victim or victims family, if the defendant has been released, applies for a pardon or parole or transfers to a release program. - Personal Injury Compensation
This form should be used by a victim of a violent crime such as sexual assault, DUI, hit and run, or robbery with injury. This will allow the victim to apply for compensation and benefits that cover loss of wages, mental health counseling and medical expenses. - Survivor Benefits Application
This form should be used by victims of a crime, or their surviving family members or dependents to apply for compensation and benefits from the Crime Victims' Compensation Program. The victims of violent crimes may apply for compensation and reimbursement to cover unreimbursed medical expenses, mental health counseling, funeral expenses, lost wages and some court costs. - Notice of Right to Request Waiver of Deductible
This form should be used by a victim of a crime to request a waiver of the $100 deductible that is deducted from every claim that receives compensation by the Office of Victim Services.
