  This notice is to inform you that a motion to amend is no longer necessary when adding/deleting creditors to schedules D, E, and F or the matrix.

• Petition for Admission Without Examination
  Use this form to petition the court for admission to the U.S. District Court for Rhode Island without taking an examination.
• Exemplification Certificate

• Statement of Military Service
  Parties to a bankruptcy case who might be eligible for relief under the Servicemembers’ Civil Relief Act of 2003, Pub. L. No. 108-189, should complete this form and file it with the Bankruptcy Court. (Form 202 (08/06)
