• Erie County Pending Litigation (Custody) Affidavit
  This Pending Litigation (Custody) Affidavit form is to provide the court with information regarding the child's residence, and a sworn statement stating that the child is not being adopted by another person, and that there is no knowledge that another party has custody or claims to the child.
• Erie County Petition for Adoption of a Minor
  This Petition for Adoption of a Minor is to be used to petition to the court the adoption of the named minor. This petition requires the signed permission of the person or agency consenting to the adoption.
• Erie County Judgment Entry Setting Hearing and Ordering Notice
  This Judgment Entry Setting Hearing and Ordering Notice is to be issued by the court to set a date for the hearing on the adoption of the named child.
• Erie County Notice of Hearing on Petition of Adoption
  This Notice of Hearing on Petition of Adoption will be issued by the court to set a date on the hearing of the Petition of Adoption or name change of the named minor.
• Erie County Final Decree of Adoption
  This Final Decree of Adoption is issued by the court stating that everything that is required to adopt a minor has been successfully completed, and therefore the adoption of the minor is ordered and that the name of the child will be changed.
• Erie County Adoption Certificate for Parents
  This Adoption Certificate for Parents will be issued to the adoptive parents, stating that all has been completed in the adoption of the minor child. This is certificate is to document the adoption with the courts.
• Erie County Vital Statistics Certificate of Adoption
  This Vital Statistics Certificate of Adoption is to be used to supply the department of vital statistics with the certificate of adoption of the named minor child.
• Erie County Petition for Approval of Placement by Adopting Party or Parties
  This Petition for Approval of Placement by Adopting Party or Parties will be used to request an investigation into the petitioners of adoption of the unborn child. This form also states that the Petitioners acknowledge that there will be prenatal, medical and hospital expenses incurred by the surrendering minor.
• Erie County Application for Proposed Placement
  This Application for Proposed Placement should be completed by the surrendering mother, to apply for the approval of the approved placement of the unborn/born child for adoption.
• Erie County Affidavit of Rights
  This Affidavit of Rights is used for the surrendering mother to sign, stating that she understand the rights that she has when deciding to relinquish her child.
