- Uniform Adoption - Attorney's Affidavit (Agency & Private Placement)
Prior to the entry of the adoption decree, every attorney shall file this form declaring all compensation that he/she will receive for adoption services. (Form 836). - Petition For Adoption (Agency)
Use this form to formally request a court order approving the adoption of the adoptive child (when using an adoption agency). (Form 1-A). - Verified Schedule (Agency)
Authorized adoption agencies use this form when placing an adoptive child in an adoption. The agency will give information on the birth parents, sibling information, the medical history of the child, consents to the adoption, etc. (Form 1-B). - Petition for Adoption (Private Placement)
Use this form (under a private placement) to formally request a court order approving the adoption of the adoptive child by the petitioner(s) and directing that the adoptive child shall be treated in all respects as the child of the petitioner. Information on the petitioning adoptive parent(s), the adoptive parents, the birth or legal parents of the adoptive child, the child's heritage, petitioners income, religious faith, etc. all rendered here. (Form 1-A). (Form 1-C). - Affidavit (Change of circumstance since certification as qualified adoptive parents) (Private Placement)
Use this form when there is a relevant change of circumstance after the court certification as qualified adoptive parents (under a private placement). (Form 1-Ca). - Child's Medical History
Use this form to give information to the court on the child's medical history and the medical history of the birth father and birth mother. (Form 1-D). - Affirmation Regarding Venue
Use this form to request another court in a different county to retain jurisdiction over the adoption proceedings. This form is submitted in cases where the family has moved to another county, the guardian of the child is in another county, the adoptive parents are in another county, etc. (Form 1-E). - Determination of Family Court Judge Regarding Venue
This form is used when the court jurisdiction has changed. The form will be submitted by the Family Court judge who heard the most recent adoption proceeding so the latter judge can then incorporate the determination into the Order Regarding Venue. (Form 1-F). - Order Regarding Venue
The judge uses this form to order that the adoption proceeding shall be heard in that court or order that the adoption petition regarding the child be transferred to another court. ( Form 1-G). - Agreement of Adoption & Consent (Agency)
The petitioning adoptive parent(s) (using the services of an adoption agency) use this adoption form to agree to adopt the child and to treat the child in all respects as his/her/their own lawful child and to extend and assure the child all the rights, benefits and privileges incident to such relationship. (Form 2-A).
