Michigan Adoption Law Forms

  • Advice of Rights After Order Terminating Parental Rights
    This form should be used by parents that have given up parental rights to a child. This form will describe the rights that these parents have including the right to request for a rehearing in the circuit court, an appeal and the right to control the release of identifying information.
  • Certificate of Adoptive Information
    This form should be used when an adoption is to take place. This form requires the information about the biological parents of the child, the names and addresses of the adoptive parents and the name of the adoptive agency that has information regarding the adoption.
  • Consent to Adoption by the Adoptee
    This form should be used by the individual to be adopted. This form will document that the consent that has been given for the adoption and to acknowledge the rights of the adoptee.
  • Consent to Adoption by Agency/Court
    This form should be used by the adoption agency or court to consent to the adoption of the named child. The consenting agency must be an authorized Michigan Department of Human Services or the family division of circuit court having permanent custody.
  • Consent of Adoption by Parent
    This form should be used by the child's mother or father to document that the parents of the child understand his/her rights when consenting to the adoption.
  • Consent to Adoption by Guardian
    This form should be used by the child's guardian to document that the guardian of the child understands his/her rights when consenting to the adoption.
  • Declaration of Inability to Identify or Locate Father
    This form should be used by the mother of the child. This form will document the name of the father or the inability to identify the father of the child. This form will list all of the ways that the mother has attempted to locate the father.
  • Motion and Order for Delayed Registration of Foreign Birth
    This form should be used by the adoptive parents of a child that was born abroad. This form will request that the court file with the Michigan Department of Community Health, the delayed registration of a foreign birth, or the that the court determine the date and place birth of the adoptee.
  • Petition by Parent/Guardian for Return of Child and Ex Parte Order
    This form should be used by parent or guardian to request that the court revoke the temporary placement and order that the prospective adoptive parents immediately return the child to the mother.
  • Petition for Adoption
    This form should be used by the prospective adoptive parents (step parent adoption), requesting that the court grants the adoption of the named child.
