  Use this Appearance Waiver and Consent to Adoption for the natural parent of a minor child to waive the right to be present in the proceeding of terminating parental rights of the minor child. This form also includes a consent to adoption form. This form is to be filled out and signed by the natural parent of the minor child giving informed consent to the voluntary adoption of the child. This form is to be used in the state of Kentucky.

  Use this Petition to Inspect Adoption Records to request permission from the Kentucky Court to inspect adoption records of the Adult Adopted Person (Age 21 or older) retained by the court.
