• Final and Irrevocable Consent to Adoption by a Specified Person or Persons
  Use this form to give final and irrevocable adoption rights for your child to a specified person. (CFS 403)
• Final and Irrevocable Consent to Adoption by a Specified Person or Persons Checklist of Understanding
  This form should accompany the final and irrevocable adoption rights form and indicates your understanding of the situation. (CFS 403-A)
• Affidavit of Identification
  Use this form to identify yourself and state that you understand the consequences of your actions in the adoption proceeding. (CFS 403-B)
• Adoptive Parents' RIghts and Responsibilities in Illinois
  This form lists the rights and responsibilities of the adoptive parents of a child being relinquished for adoption. (CFS 403-D)
• Report of Investigation
  This form is used by the DCFS to do a final investigation before the permanent adoption decree is ordered. (CFS 411)
• Application/Record of Child Information
  Use this form to give the DCFS information about your child when you place them for adoption. (CFS 428)
• Final and Irrevocable Surrender to an Agency for Purposes of Adoption of a Born Child
  Use this form to give final and irrevocable adoption rights to the DCFS for your child. (CFS 435)
• Adoption Listing Service Family Registration Agreement
  Use this form to register as a waiting family with the Adoption Listing Service of Illinois. (CFS 448)
• Adoption Listing Service (ALS) Child Registration Form
  This form is used by the DCFS to send information on a child to the Adoption Listing Service. (CFS 468-1)
• Adoption Listing Service Listing Eligibility Form (ALS-1a)
  This form is used by the DCFS to determine if a child is eligible for the Adoption Listing Service. (CFS 468-1a)
