Hawaii Adoption Law Forms

• Adoption Decree
  This form is used by the court to issue a final decree of adoption for a child.
• Adoption Hearing Flag Sheet
  Use this form to give the court relevant information regarding consents received, documents filed, etc. for the adoption of a child.
• Adoption Information Sheet
  This form is completed by the petitioner and gives the full name of the adopted child, the adoptive parents and the natural parents.
• Adoption Procedures and Forms Memo
  This adoption memo lists the court fees involved, and the documents that must be filed for an adoption.
• Affidavit of Adoptive Parents
  Use this form to certify to the court that you have been advised of the statutes regarding confidentiality of the court's adoption files and that you will inform the child.
• Consent by Child (Age 10 or Over) to Adoption
  This form is signed by a child 10 years of age or older to signify that he/she agrees to his/her adoption.
• Ex Parte Motion and Affidavit for Custody of Child(ren) Pendente Lite; Order
  Use this form to petition the court to grant temporary custody of the child(ren) until the adoption is settled.
• Findings and Decisions of the Court Granting Petition for Adoption
  This form is used by the court after a hearing to issue its findings and, if decided upon, a final decree of adoption.
• General Consent to Adoption
  This form is completed by the child(ren)'s natural parent(s) to give his/her consent to an adoption.
• List of Exhibits
  Use this form to present to the court any information or documents relevant to the adoption.
